To Register your Team(s) for the U.S. National Championships Tournament follow the steps below:
Step 1: Download the Player/Team Roster Spreadsheet
Step 2: Fill out sheet electronically and email electronic copy to: e.a.wehmeyer@gmail.com before May 19th, 2017.
Step 3: Print off Roster Spreadsheet, Mail Spreadsheet and waivers along with entry fee cheque ($1,100 Team Fee + $5/player USOA Fees + $35/each banquet ticket) to:

All entries must be received before May 19th, 2017 to avoid $100.00 late fee penalty. No entries will be allowed after June 2th, 2017.
Step 4: Completed USOA Waivers, Team Cincy Waiver, Team Cincy Media Release Form must be signed for each player and received prior to play on Friday. No Waivers – No Play – No Exceptions.
